The fallacy of the routine medical check-up: part 2

Central to our belief in the “check up” is the idea that early detection must be good. The strength of construct validity of this idea has overwhelmed evidence to the contrary.

Perhaps science fiction is to blame: Doc McCoy’s pocket scanner certainly reflected that widespread belief that only if we could see inside the human body then we could pick up diseases early enough to “nip them in the bud”.

Maybe its deeper in our psyche.  The early bird gets the first worm. Nip it in the bud. 

Different cultures have the same concept. In this country, Maori (indigenous Polynesians) have a proverb which says “The first come to eat food, those who are behind swallow spittle.”

The problem is that the research on the routine medical check up usually does not focus on true outcomes, because it is too remote from this although for specific realms of medicine there are intermediate benefits e.g. cardiovascular and diabetes. However as for listening to the health y heart, palpating the asymptomatic abdomen, or inspection of the unsymptomatic urine, then the data is either lacking or unsupporting.

Microscopic haematuria is a classic. No use as a screening tool in fact probably downright dangerous for the high probability of a positive result is matched only by the high probability of an unhelpful or incurable finding following invasive investigation. And as for prostate screening, well the Americans found it didn’t save lives and the Europeans found it did but at a very significant cost in terms of adverse patient outcomes.

But we still do it and the tea leaves suggest this will not change.

That’s medicine.

Its very nice to find this blog and share troubleshooting of health sistem in Bulgaria last 9 years.
About check ups. The good doctors practice according my point of view can be flexible and fit to the individual case. Yes, we should follow guidlines which are so so common nawadays, but we must follow our acceptance first to care in best way for the patient.
We are in health reform during last 9 years. What is bad ?- that there is too many changes in primary care including obligatory check ups and this make us in bad possition not abel to care properly to our patient. This make us put in situation not to be able to do some needed according the case check ups and do too mush not in use check up ,which are obligatory for prevention or obligatory for monitoring some chronical diseases.
It is too bad to work in such a conditions and to have good care for the patients

One of the hardest problems is preserving the care for our patient from being controlled by the funders. It seems that where the funders are insurance industry the routine checkup is very very strong. The problem is that in other countries the governments/national funding also can control the patient care such as in the UK, but at least this is often more evidence based and focused on a population need.
Who funds primary care funding in Bulgaria?

Isnt it natural for us to believe we are healthy and not suffering from any disease ? I had a similar thought process until my physician asked me to get a heart scan done after he found that my basic cardiograms were not perfect. This heart scan screening has been some cause for concern, both in terms of usefullness and also in terms of the harms.
The following study

showed that the coronary artery calcium score did not seem to add much beyond risk factors alone for prediction of coronary events.
..it is a lot cheaper to do the risk factor calculation than the scan and a lot safer because there is no radiation exposure in doing the cardiac risk factor calculation.  
In this country patients have to be fully informed on the harms and benefits of such scanning before they have the test.

Just to clarify:
the "routine medical" is the doctor visit by a healthy person who is free of symptoms in order to detect that there is something wrong with their healht (i.e. to search for a disease). it isn't about a regular check to make sure a persons actual disease or condition is being managed properly.

It's difficult to agree or disagree with any of this. In my Biology of Women class, my professor made it clear that yearly* physicals/checkups are key to early detection. However, most doctors don't check if you don't ask them to. For example, ovarian cancer is known to be a silent killer for women, but nobody asks, so the doctor doesn't have to do it. I think that's ridiculous. That's what doctors are for, right? They know what to look for; that's why they were in medical school for 8,12,16 years (still boggles my mind).
On the other hand, check-ups every two months seem ridiculous. If your body is telling you something's wrong, by all means, but unless you feel the need to go to a doctor, why spend the money? As a college student, I know that I don't have extra ;)

Actually, I've heard that as well. And experienced that. When I go for my yearly physical, I make sure to scribble out a list of things that were bothering me throughout the year. If I don't mention them, my doctor doesn't ask. I guess it would also depend on the patient-doctor relationship.

Sorry everyone. As this blog is designed for doctors, I have made presumptions about readers  knowledge of the harms and benefits of screening.

The truism about screening (that is the sort of thing that you get in a regular check up when a patient doesn't have any symptoms of anything wrong) is that
"all screening does harm, some does good"

Ovarian cancer is a classic: screening for this condition does more harm than good, something that has been established in research trials. All you do is find a whole lot of funny things in women pelvic areas that are not cancer, but you have to deal with them and all the worry, operation side effects etc.  that that produces.
Never rely upon opinions that are not supported by research. It is important to get away from opinion and actually use the research to make decisions about health care, for there is an abundance of opinions in medicine that have been proven wrong, by research.  

The concept that "finding it early" might cause more harm than good is very very difficult to get over, so ingrained is the concept in Western Society. A paradigm shift is required in order for us to get a better understanding of disease progress especially for some cancers.

Hello, Johhny, I get the feeling that quite a few of the posts are actually low paid people spamming.  We have now removed all links back to the spammers sites, their pseudo names, product promotions etc so it's now a waste of their time to post spam.  As it's now a completely fruitless experience for them, most won't eventually bother as they could be spamming other websites and getting actual link backs to their spam sites.  We will instigate validation it we continue to get spam and delete ANY post with self promotion.  Automatic spamming by robots is already taken care of.
